Discharge Instructions

We require a responsible adult to accompany our sedated patients.  Hopefully this is the same person caring for the patient that day.  We verbally review the written post-op instructions, have the escort (not the sedated patient) sign the instructions and then send a copy home.  We also make both the patient and escort aware they can phone us for questions and we call later that day to check on the patent's condition.
Patients receiving local or nitrous and local can sign their own instructions.
If the DPOA or escort from an outside facility are with the patient they can sign the instruction sheet.
