Intraop fire risk assessment

Our facility is needing a "fire risk assessment" check off for our intra-op sheet.  If anyone has one they are using and can send it to me it would be greatly appreciated.

I would love one, too. That is the only Partial Compliance we received on our recent AAAHC survey--no fire risk assessment.
