Our facility checklist involves each department (pre-op, Intraop, Anesthesia and PACU. Each department is responsible for their section. Audits are done to ensure completion. The checklist is part of the pts. medical record. If you would like a copy of the checklist, please let me know.

Our facility also keeps the completed checklist in the medical record.

We put a Safe Surgical Checklist Completed check box on each patient chart and added a signature line to our surgical form.

Our facility is currently using a checklist that goes directly into every patients chart
