This Excel document includes a list of all survey questions (by section) and can be shared with others to assist in gathering data needed to complete the survey. Data needed to complete the survey includes: facility profile and demographic information; annual salary and bonus information for management positions; annual salary information for clinical and non-clinical positions; and employee benefit program details for benefits offered to employees, families and domestic partners.
